Deglycyrrhizinated licorice, known as DGL, has been employed in clinical settings for many years, primarily to address peptic ulcers. Licorice is rich in numerous triterpenoids and hundreds of flavonoids that exhibit inhibitory effects on various inflammatory enzymes, such as cyclooxygenase-2 (COX-2) and 5-lipoxygenase (5-LOX). These enzymes play a role in reducing the production of potent inflammatory compounds like IL-6, prostaglandin E2, thromboxane B2, and leukotriene B4. It's crucial to emphasize that the removal of glycyrrhizic acid, a component associated with mineralocorticoid excess and hypertension, in DGL does not compromise its inhibitory properties.

Licorice encompasses a range of active compounds with anti-inflammatory, antimicrobial, and anti-allergic properties, including glabridin and a minimum of seven licochalcones. Research indicates that these compounds can prevent the production of inflammatory compounds induced by lipopolysaccharide (LPS), impede the formation of yeast and bacterial biofilms, and stimulate antioxidant enzymes.

In clinical studies, DGL has demonstrated effectiveness comparable to cimetidine in both the healing and prevention of ulcer recurrence. Additionally, licorice extracts have exhibited inhibitory effects on the adhesion of H. pylori to the gastric mucosa and the growth of antibiotic-resistant strains. This suggests that the anti-ulcer effect of licorice operates through multiple mechanisms. The use of chewable tablets is deemed essential for the efficacy of DGL, facilitating distribution and absorption through the gastric mucosa.
